Video-assisted thoracoscopy surgery lobectomy for lung cancer T1NOMO

ABSTRACT
Background: Video-assisted thoracic surgery, which has developer rapidly in the two past decades. The study was evaluate the outcomes of a video-assisted thoracoscopy surgery lobectomy for lung cancer, with a special focus on its loco-regional control, were compared with a conventional lobectomy. Methods: We performed review of 80 patients who had undergone a lobectomy either by means of thoracoscopy surgery (n = 39) or a standard thoracotomy (n = 41) for clinical T1N0M0. No small-cell lung cancer. Results: The total recurrence was 17.5% (8 loco-regional and 6 distant). In the 8 documented local recurrences, 6 belonged to the thoracoscopy surgery group and the standard thoracotomy group, without significant differences (p = 0.229). The overall 5-year survival rates associated with the thoracoscopy and standard procedure were 89.1% and 77.7%, respectively (p = 0.149). No significant differences in the disease-free or loco-regional recurrence-free survivals were observed between the groups. The results of a multivariate analysis for the incidence of total and loco-regional recurrence demonstrated that covariates, lymph node metastasis and surgical side were significant factors for total and loco-regional recurrence. Conclusions: Thoracoscopy surgery is not inferior regarding its ability to achieve loco-regional control in comparison with standard procedure.
