ABSTRACT
The pericardial cyst is a rare presentation, with the incidence of 7% from all the mediastinal mass cases. Most cases present the cyst at the right angle of the heart and the middle mediastinum, frequently asymptotic, an exception in those patients with symptoms who present the cyst outside the places described. About the symptomatic patients, the position and localization of the cyst and the type of organ in relation will determinate the kind and presence of clinical manifestation. We present a 39 years old female patient who presented a vena cava syndrome as a first presentation on pericardial cyst. The patient was approached with simple chest X rays and CT scan, with evidence of superior mediastinal mass. The patient underwent to biopsy by percutaneous fine needle aspiration cytology with diagnostic of tymoma. We underwent the patient to surgery to anterior approach by medial sternotomy for exploration, with the finding of cystic mass that caused superior vena cava extrinsic compression. This is a very rare presentation of pericardial cyst, also we consider to include the pericardial cyst as differential diagnosis in the superior vena cava syndrome etiologies. We conclude that in cases with clinical manifestation of pericardial cyst at any localization or presentation, the patient have to consider under some modality of treatment. The therapeutic options include percutaneous needle aspiration, mediastinoscopy, thoracotomy and thoracoscopy.KEYWORDS
