ABSTRACT
Pulmonary sarcoidosis is a multisystemic disease of unknown etiology; it affects the lungs and lymphatic nodes, with variable involvement of extrapulmonary disease associated, the diagnosis is based on the findings of a granuloma in lung biopsy, lymphatic nodes, skin, or others organs affected; However, in asymptomatic patients with typical data, the radiographic and clinical diagnostic is enough and the close evolution following is necessary. Systemic corticosteroids is the therapy in use actually but indications are individualized in each case, pulmonary sarcoidosis in stage II and III, the use of systemic steroids do not have doubts specially with persistent or increment in the radiographic infiltrates, presence in the pulmonary function tests of moderate to severe alterations and deterioration, instead inhaled steroids continue been a controversy aspect in medical management in this entity and it’s use is only in mild forms specially when cough or bronchial hyperactivity exist as the unique finding.
