The COVID-19 pandemic, caused by the SARS-CoV-2 virus, presented an unprecedented challenge to global health systems. The National Institute of Respiratory Diseases Ismael Cosío Villegas (INER) in Mexico, as a tertiary referral center, consolidated its treatment protocols based on precision medicine and accumulated clinical evidence. This article describes the therapeutic approach for patients with severe Acute Respiratory Distress Syndrome (ARDS), ranging from the use of broad-spectrum antivirals, immunomodulatory therapies, the management of thrombotic complications and the impact of vaccination on the clinical spectrum of the disease.
